When you or a loved one are planning to move to one of our centers, there are many issues to consider. We will work with you to make the transition from home to your new residence as smooth as possible and help you to understand the admissions process. If you ever have any questions, please contact us and we will guide you through preparing for a stay at our center.
Our team is composed of many individuals who have your best care in mind. From the Executive Director, Director of Nursing, nursing staff, Community Liaison and our Business Office Manager, we are here to listen to any concerns you have about your stay. We encourage you to be open about any clinical issues with our nursing staff and to ask any questions you have about your care or financial obligation. We are here to help you both at pre-admission and during your stay. Please let us know what we can do for you.
The Admissions Process
The admissions process begins before the resident makes it through the front door. In fact, we are preparing for the stay immediately upon notification that a new resident will be staying with us by developing a plan of care prior to admission. Before you or your family member arrives, we are preparing the room for any medical needs and developing a plan for rehabilitation, recovery, pain management and more.
Upon admission, a member of our team will meet with you and your family to gather important information to develop your clinical plan and to ensure proper billing is completed.
At your admissions meeting, please provide the following items:
- Medicare and/or Medicaid care
- Medicare Part D (drug benefit) card
- Managed care or insurance card
- Social Security card
- Advance directives, living will, etc. (if applicable)
- Clinical and/or financial Power of Attorney documents (if applicable)
- Long term care/supplemental insurance policies
- Name of person to be contacted with change in condition/financial status
- Emergency contact information
- Healthcare proxy information
Our admissions team will review the documents provided and explain all amenities and services at our nursing and rehabilitation center and answer any questions that you may have. You will also meet with our business office to discuss your financial obligation.
The Clinical Admissions Process
Within 72 hours of admission, our clinical team will meet with you to perform a clinical evaluation that will be used to refine the care plan based on your unique medical needs. This will include information about your diagnoses, medications, recent hospital stays, and current activity level. At this time, all vital information will be recorded. It is at the meeting that our team will consult you and your family to determine objectives for care and the best treatments and activities to achieve these goals.
Please provide the following information:
- Current or previous health history including surgeries and treatments
- Allergies list
- Vaccination record
- Current medications, including dosage
- Recent diagnostic results
- Health care provider names and contact information
Members of our clinical staff meet with each resident on an ongoing basis to assess changing clinical and psychosocial needs, lifestyle and discharge plans. Members of our team that may meet with you include our Case Management Coordinator, Activities Director, Rehabilitation Coordinator, Registered Dietitian and Social Services Director.
Together, you, your caregiver and our team will identify needs that need to be accomplished during your stay. Your treatment will be built around these needs in order to achieve discharge goals.
Our center offers many professional services; however, if you would like to use a professional outside our services you have the option to do so. If choosing a professional services provider, please bring their name, address and telephone number with you on the day of admission. All outside service providers must comply with all applicable rules and regulations.
- Physician – one who is available to visit you at the center as required by licensure/certification
- Alternate physician
- County caseworker
- Church/clergy name
We want your space to be a reflection of you and your home. We encourage you to bring personal items from home as space allows such as pictures, quilt, and small pieces of wooden furniture. Please label all personal belongings with your name. You are welcome to arrange your side of the room as you would like. Please have all items placed on resident's inventory form. If there is an item taken home please have this documented on the resident's inventory form.
Packing for Your Stay
Packing for your stay is an important part of getting ready for admission. We recommend that you bring 7-10 days worth of comfortable clothing and shoes that are suitable for rehabilitation activities. You may bring extra clothing if you would like to change throughout the day. Please mark all clothing with written labels or printed in permanent ink that is sewn into the item.
The following is a suggested list of items to bring:
- Dresses, slacks, shirts or blouses
- Undergarments, undershirts and shorts
- Comfortable shoes and slippers with traction
- Socks or pantyhose
- Pajamas and robe
- Sweater or light jacket
- Coat, hat and scarf (seasonal)
- Hearing aides
- Personal hygiene supplies (please label)
